The ABC Board Warehouse is responsible for receiving, sorting, and shipping equipment and supplies to ABC stores. Upon request from the Stores Operations Division to transfer equipment from the Warehouse to a store, a Supplies and Equipment Invoice (Form M-5) is to be prepared by the ABC Property Inventory Officer to authorize the shipment. Information on the invoice includes equipment/supplies quantity, description, and state property number. The invoice is then forwarded to the Warehouse for shipment. The original copy and triplicate, to be signed by the store manager upon receipt of shipment, are maintained in the Accounting Division while other copies are retained in the Warehouse, the ABC gaining store, and truck carrier.
